SCSC Health Insurance Staff Needs Your Help!

One of the challenges South Central Service Cooperative faces is to get information to the correct people in a timely manner. We would like you to take a moment to let our staff know who you want listed as the contact person for various health insurance notifications. We have posted a form on our website for you to fill out and return to our staff. Please take a moment and indicate the appropriate contact person for each area. Make sure to include the contact’s name, title, phone number and extension, fax number, e-mail address and any additional information that will help South Central staff communicate with you. This form will remain on our website for any future changes to your staff or changes to contact information for existing staff.
