This voluntary program is offered through National Insurance Services and underwritten by Delta Dental of Minnesota. Rates are guaranteed for 12 months from your effective date and are also pooled with participating members from the Participating Minnesota Service Cooperatives. The employer has three plans to choose from – and once that is determined, the employee has two options to choose from. An employer needs only 5 employees to purchase this coverage in order to make it effective. There is not any waiting periods for benefits to begin.
